It doesn't really matter exactly how you hold it. The idea is that on each bead you say a specific prayer, then move onto the next bead, etc. The physical beads are only really there so you don't lose track of where you are within the overall prayer of the rosary. The same spiritual benefits can be had without the beads, but it just makes things a heck of a lot easier. Any imagery on there can also aide in prayer.
